October 1, 2011 DEADLINE For New HHS/DOT Custody and Control Form (CCF)

We are less than six months away from the deadline for mandatory use of the new Federal DOT Custody and Control Form (CCF). The CCF that was in place prior to the October 1, 2010 changes to the Mandatory Guidelines remains valid until September 30, 2011.

NOTE: After September 30, 2011 if an old CCF is used, laboratories must treat it as a correctable discrepancy and request a memorandum for record (MFR).

Please inspect your supplies to ensure that you have the proper CCF for use on the October 1st deadline. We will send out new CCF forms upon request.
